Daily monitoring is the routine process of observing, recording, and evaluating key activities and conditions within an organization or facility on a daily basis. In the context of IQ Kare and similar health or homeless service programs, daily monitoring helps ensure smooth operations, client safety, and accountability.

Purpose of Daily Monitoring

• Ensure consistent service delivery (medical care, case management, meals, etc.)
• Track client movement (intakes, discharges, transfers)
• Monitor staff coverage and shift activity
• Identify urgent needs (medical, behavioral, facility issues)
• Support quality improvement and compliance (especially for funders and auditors)

What It Typically Includes

Client Updates

• Total number housed
• New admissions
• Discharges
• Critical incidents (e.g., hospitalization, behavioral crisis)

Staffing Overview

• Who is on shift
• Any shortages or overtime
• Notes on performance or support needed

Medical & Behavioral Health

• Any medical alerts or issues flagged
• Behavioral incidents or interventions required

Facility Operations

• Maintenance issues
• Security concerns
• Supply shortages

Program Services

• ECM or case management sessions completed
• Referrals made
• Progress toward discharge plans

Notes & Follow-ups

• Any actions needed
• Coordination with outside agencies (hospitals, social workers, etc.)
