Post-Hospital Recovery Services are at the core of what we do at Industry Qualitative Kare Group. Whether individuals come from hospitals, treatment centers, or homeless shelters, our mission is to provide comprehensive care and recovery support for displaced patients across the LA/Orange County area. Studies show displaced individuals experience hospital stays that are four days longer on average—making post-hospital recovery services essential to their healing and reintegration.
At Industry Qualitative, every patient receives a full-spectrum follow-up care plan. Our post-hospital recovery services include medical oversight, wound and injury care, and document readiness. We assist in linking patients to A Bridge Home, connecting them with housing navigators to continue their path to permanent housing. We also coordinate transportation to appointments and pharmacy pickups, ensuring no detail is overlooked.
Mental health support is another key part of our recovery model. We provide assessments, one-on-one discussions with licensed mental health professionals, and referrals for long-term care. Our team also collaborates with families to build custom care plans that ensure a successful transition back into the community.
Through our work, we aim to address Social Determinants of Health (SDOH) and improve overall quality of life. As part of the CalAIM initiative (California Advancing and Innovating Medi-Cal), we align with state goals to improve care access, enhance provider coordination, and reduce long-term healthcare costs.
